Erection problems, which are expressed as Erectile dysfunction (ED) in the medical literature, mean the inability to initiate and maintain penile erection, which is needed for sexual intercourse in men. Impotence is one of the most common sexual problems reported to urologists by men. It is known that 30 million men worldwide have erection problems. Men with erectile dysfunction experience problems in obtaining a firm erection, early termination of an erection, and maintaining an erection. Although it is normal for all men to have problems with erection from time to time, if this problem starts to occur in all sexual intercourse and continues for a long time, this should be defined as an ED and treated.
In a study conducted throughout Turkey, the distribution of erection problems according to age ratios has emerged as follows. As can be seen from this table, erection problem is a common ailment that is not underestimated.
In the absence of sexual arousal in men, the penis is loose and soft. Erection occurs in men who are sexually stimulated as a result of visual, auditory or contact stimuli. When men are sexually aroused, blood flow to the penis begins to increase. The blood flows into the two erection chambers of the penis, called the corpus cavernosum, which are made up of muscle tissue. During erection, the tissues in these chambers relax and trap blood here. After the man's orgasm, a second signal goes to the penis, and as a result of the contraction of the tissues there, the trapped blood returns to the circulation. Thus, the erection ends and the penis returns to its normal state. This cycle that occurs in the penis is called an erection. In order for men to be ready for sexual intercourse, the penis must be in an erect state.

One of the most important causes of erectile dysfunction is the stress experienced during sexual intercourse or the emotional problems experienced periodically. However, erection problems experienced due to stress or emotional reasons are often short-lived and do not require treatment. In addition, one of the most important factors that cause erection problems is insufficient blood flow in the penis and the nerves in this area are damaged for any reason. In addition, other conditions such as atherosclerosis (hardening or blockage of the arteries), heart disease, high blood pressure or high blood sugar from diabetes can also be among the causes of erectile dysfunction.
Erection problem, in other words impotance, can be caused by some psychological and physical reasons, and in some cases both can affect the formation of erection problem. Some risk factors that can cause erectile dysfunction are:
Although erection problems become more common in men as they get older, erectile dysfunction does not always occur as they get older. In some men, sexual functionality can continue until the age of 80. In addition, erection problems can also be a sign of other health problems. For this reason, it is an important step to consult physicians to find and treat the cause of erection problem.
It is seen that men who think they have erection problems sometimes do not share this situation with anyone, and they only search on the internet for foods that are good for an erection, drugs that are good for an erection, or vitamins that are good for an erection. First of all, it should be accepted that erection problem, which is such a common urological disease in the world, is not something to be ashamed of. Just as we apply to doctors for any physical ailment, we think that we have such a disorder and experience its symptoms, we must first see a urologist. It should not be forgotten that other diseases such as diabetes and high blood pressure can also cause erection problems.
Erectile dysfunction can have many different causes, both psychological and biological. For this reason, it has to be a multi-dimensional examination phase with the patient's application due to erection problems. First of all, physicians listen to the patient's history in order to determine how long the patient has had this problem and whether the patient has any other chronic diseases. Then, the physical examination of the penis and testicles and the sensitivity of the nerves are checked by physicians. The patient's blood and hormone tests are performed and the male hormone level is measured. In addition, penile color Doppler ultrasonography is performed in order to observe the blood circulation to the penis. Thanks to this observation made with ultrasound, the condition of the vessels in the patient's penis and testis region is observed.
If a biological basis for the erection problem experienced cannot be determined with these tests, it should be evaluated that the situation is caused by psychological reasons and psychological support should be provided to the patient. For this reason, if possible, a therapy process is initiated with the patient's spouse.
Erectile dysfunction is a disease that affects more than 50% of men between the ages of 40-70. For this reason, there are many studies in the medical world on the treatment methods of this disease. As a result of the studies, rapid progress has been made in treatment methods.
Today, with the developing technology, it has become possible for patients who have erection problems to completely get rid of this disease without pain. After the cause of the erection problem is determined, there are different treatment methods applied in the light of this reason. The most common of these methods, of course, is Shock Wave Therapy, or ESWT therapy, which is determined entirely for the patient.
Shock Wave Therapy, which was first applied in the treatment of urinary stones in the 80s in the medical world, is a method used mainly in the field of orthopedics. However, studies have shown that the ESWT method provides permanent benefits in chronic wounds, cardiac ischemic tissue, peripheral neuropathy and erection problems. This method has become very common in the field of medicine in the last 20 years due to its advantages such as the absence of any side effects and permanent solutions.
This method is applied after concluding that the erection problem is caused by the patient's vascular access that provides blood flow to the penis. In this method, the vessels in this area are stimulated with high pressure sound waves sent to the patient's penis area and it is aimed to accelerate the blood flow to the penis.
The Shock Wave Therapy method is a very reliable method that has been used for the treatment of different diseases for a long time. It is a very important advantage that the patients do not feel any pain in the ESWT treatment, which has been found to permanently correct the erection problem in many studies, and that they can continue their normal lives during the treatment.
This method, which is applied by transmitting high pressure sound waves to different points of the patient's penis area, takes a total of 2 weeks. During this period, ESWT is applied to the patient at intervals of 48 hours. In this process, the patient can continue his normal life and there is no problem in having sexual intercourse. ESWT is applied by specialist urologist physicians in the clinical setting and the patient does not feel any pain due to the application of local analgesic cream on the penis during the application. In practice, shock is applied with sound waves to different points of the penis by means of a special probe. A session lasts an average of 15-20 minutes, and the patient continues his normal life after the session. Although the results of the ESWT treatment may differ from person to person, it takes an average of one month. However, during this period, patients are expected to pay attention to their diet and exercise and to stay away from smoking.
